This program is offered to Erie area Realtors and Contractors by John V. Furniture and Mattress, the leader in home furnishings and interior design.

John V. Schultz Furniture and Mattresses exclusive Partner Awards program was created to thank you, our partners, for referring your clients to us and to enable us to offer more personal service to your special clients as follows:

1. Complete and fax us the application on the back of this form.
    
2. After joining you can access our Client Registration forms from our website (see step 3 of the Partner Awards page) to enroll your clients upon signing a new home contract or performing a service. All sections of this form must be completely filled out by the you, the Partner, or it is invalid!(One Client Registration Form per residence, please.)
    
3. Clients are automatically registered and linked in our computer to their partner when we receive this form. Once your clients are registered they will receive an additional 6% off of their next purchase in the store (valid up to 6 months, applicable to one purchase only).
    
4. Award points equal 2% of all client purchases and may be spent like cash at John V. Furniture.
    
5. Award points are tracked automatically by our computer, linking our Partners to their clients.
    
6. You can view your points simply by using the "Log In" feature on our website.  Your accumulated points will prominently display at the top of the page.
    
7. Referral must have purchased a home in the Erie Area within the past 6 months to be eligible.

Our goal is to make it as easy as possible to send referrals to John V. Furniture and Mattress and to build a lasting partnership with you. We also want you to earn wonderful new items for your home and to be able to confidently recommend us to your customers.
